- If the physician is a not preferred provider with your insurance plan, you may be required to pay the physician’s charges in full at the time of your visit.
- Take a copy of your valid medical insurance card to your doctor’s appointment.
- Notify the office staff of additional medical insurance plans you may have and have copies of those medical insurance cards available for the office staff.
- Notify the office staff of any change in your address.
Prior to your appointment, the office staff will verify your insurance. At the time of your appointment, the office staff will collect applicable co-payments, co-insurance and deductible amounts.
