- What is MyUTP?
MyUTP is a web-based tool that allows patients to communicate with their doctor’s office and access their health information at any time and from anywhere with an internet connection.
- By giving UT Physicians my email address, will I receive emails from other organizations, such as spam?
No. You will only receive emails from UT Physicians relating to your care or MyUTP. MyUTP will send emails for the following:
- An invitation to create a MyUTP account
- Verification that you have successfully created your account
- Notifications when your provider has sent you a secured message or an update to your health information has occurred in MyUTP
- I went to my appointment and was told I will receive an email to sign up for MyUTP. What email should I be looking for?
The email you receive for MyUTP registration will be from noreply@followmyhealth.com. If you do not see the email in your regular inbox, be sure to check your spam or junk folder.
- What are the benefits of having a MyUTP account?
By creating a MyUTP account, you can:
- Send and receive messages from your health provider
- Request to schedule appointments and get directions
- View and print your health records from as far back as 2008, including test results, medications, allergies, immunizations, and conditions
- Request prescription refills
- Request referrals
- View and pay your bill online
- I will not be able to access my email right away. How long until my invitation expires?
Your invitation will be active for 60 days. If your invitation expires, simply create your account from the email you received and you will send UT Physicians (UTP) a message to connect with you. Your invitation should be accepted within 24 business hours. Please refer to question 8 for additional information regarding registration without an active invitation.
- How do I complete my enrollment after receiving the email invitation?
- Select the “Register now with FollowMyHealth™” link in the email your invitation email.
- Select the “I need to sign up” button toward the bottom of the screen.
- Populate the required demographic fields.
- Select “Confirm and Continue”.
- If you agree, click on “I Accept” to the “Terms of Use”.
- Create a unique username and password (Note: It is recommended to use your email as your username).
- Click the “Connect” button next to “UTP” (UT Physicians) 6410 Fannin Houston, Texas 77030. (Note: This is the address for the UT Physicians main office. If your clinic is in the community, you should make this selection as well).
- Select “Next”.
- If you agree, sign the “Release of Information Authorization” by clicking “I Accept”.
- You will receive a “Connection Request Successful” message.
- Select “Ok”.
- You will be asked to participate in a “First Time Walkthrough”; selecting “Ok” is recommended.
- All of your health information should display within MyUTP upon the completion of this registration.
- What is my invitation code?
The invitation code is a security checkpoint when activating your account. The invitation code is your 4 digit birth year (for example: 1989).
- How do I register online for My UTP?
You can easily enroll in MyUTP from your home without receiving an invitation from UT Physicians. If you receive an email invitation from Follow My Health, please refer to question 6.
- Go to https://utphysicians.followmyhealth.com/Login/ in your internet browser.
- Select the “I need to sign up” button toward the bottom of the screen.
- Populate the required demographic fields.
- Select “Confirm and Continue”.
- If you agree, click on “I Accept” to the “Terms of Use”.

- Once UT Physicians completes the registration you will receive an email notification that your health information is now available in MyUTP.
- Why do I not see all of my information in the MyUTP patient portal?
There are several reasons that you may not be able to view your expected information on the MyUTP Portal:
- If you have just registered for an account through the MyUTP website, your enrollment may not be complete. Enrollments should be completed within 24 business hours or you can expedite your enrollment by calling 1-855-316-4256.
- Your physician or provider may not have reviewed your test result information. Following their review it will populate into your MyUTP record.
- Certain document types do not display in MyUTP. Please contact 1-855-316-4256 with any questions that you have.
- How do I print a medication and/or immunization list from MyUTP?
- Select the “My Health” tab at the top of the MyUTP Portal page.
- Select the “Medications” or “Immunizations” tab.
- Click on the “Send” action button.
- Choose “Print” from the available options.
- A message will display confirming that you wish to print, select “Print”.
- How do I pay my bill online?
- Scroll to the “Billing” section at the bottom of the home screen of MyUTP once you are logged into the portal.
- Select “Details”.
- Click the button “Pay my bill”.
- You will be routed to UT Physicians Simplee page to complete your bill payment.
- Please be sure you have your Account or Invoice Number available (both appear on your bill) to log into Simplee.
- Enter your or the patient’s date of birth.
- Click “View my bill”.
- I’ve been told since my child is between the ages of 13 and 17 I cannot access their portal information. What is the reason for this?
This change is due to laws in Texas Family Code and HIPAA which allow minors to have certain rights with regard to their health care. In order to balance your need to access some features of the portal and your child’s rights, the concept of limited access to adolescent health information in MyUTP was adopted by UT Physicians. Your child’s health information may still be accessed through our Department of Medical Records.
- My child is between the ages of 13 and 17, how can I access their information?
You are able to access your child’s health records through our Medical Records office:

To place a request with our Department of Medical Records, begin here and follow the instructions for submitting a medical record request. - I cannot remember my login information, how can I gain access to my account again?
- If you signed up with the FMH Method, select the “Forgot your password” button. You will be asked to enter your username, and you will be sent an email with instructions on how to request a new password.
- The MyUTP Help Desk can help by looking in the system to see what email address is connected to the account. You can reach them by calling 855-316-4256. If you do not know the password to this email account, you will need to go to that email provider’s website to reset your password.
- I am the caretaker for my parent/grandparent, can I sign up on their behalf?
Yes, by filling out the “Authorization for the Use and Disclosure of Protected Health Information,” you will be able to have access to their records. Submitting this form to the clinic office or the Medical Records Department will begin the process. The clinic office will be able to provide this form.
- Can I create a portal for my newborn?
Yes, please follow the steps outlined in questions 6 or 8.
- Can I un-enroll from MyUTP?
Yes, you may un-enroll from the MyUTP patient portal, though you will not receive access to your health information. Please note that your electronic health record will be maintained by UT Physicians.
