UT Physicians – Victory clinic has received Level 3 certification for providing coordinated, efficient care through the Patient-Centered Medical Home (PCMH) program of the National Committee for Quality Assurance (NCQA).

The three-year designation is granted when clinical staff prove they have combined teamwork and information technology to improve care and patient experience while reducing costs. The distinction acknowledges use of evidence-based, patient-centered processes that focus on highly coordinated care and long-term, participative relationships.

“The Victory location is the second of our community-based clinics to achieve recognition by the NCQA,” said Thomas Murphy, M.D., assistant dean of community affairs and health policy at McGovern Medical School at The University of Texas Health Science Center at Houston (UTHealth). “Achieving this high level of recognition, which less than 15 percent of clinics earn, is evidence of UT Physicians’ relentless drive to provide the best possible outcome for the patient.”

“I am proud of our commitment to deliver exceptional care,” said Latanya Sam, practice manager for UT Physicians – Victory. “The mission of the Victory clinic is to promote the individual, community and social health of our surrounding neighborhoods. We are here to make a difference.”

The NCQA has three levels of recognition ranging from Level 1 to Level 3, which is the highest. Achieving recognition requires meeting elements that include enhancing access and continuity, identifying and managing patient populations, planning and managing care, providing self-care support and community resources, tracking and coordinating care and measuring and improving performance.

According to the U.S. Department of Health and Human Services, the NCQA seal is a widely recognized symbol of quality for consumers and employers, signifying that an organization is well-managed and delivers high-quality care and service.

UT Physicians – Victory is located at 7364 Antoine Drive, Houston, 77088. The clinic offers adult and pediatric counseling, family medicine, internal medicine, obstetrics and gynecology, pediatrics and psychiatry.
