Occipital Neuralgia
What is occipital neuralgia?
Occipital neuralgia is a neurological condition characterized by chronic pain originating from the occipital nerves, which run from the upper neck to the scalp. When these nerves become irritated or inflamed, often because of compression or injury, they can produce intense, shooting or electric-like pain that radiates across the back of the head and toward the scalp or behind the eyes. Although not life-threatening, the condition can be persistent and significantly affect daily activities and quality of life.
What causes occipital neuralgia?
Occipital neuralgia most commonly results from irritation or compression of the occipital nerves. This can be caused by muscle tightness or tension in the neck, trauma such as whiplash, or degenerative changes in the cervical spine. Less commonly, the condition may be associated with tumors, vascular abnormalities, or structural issues affecting the nerve pathways. In some patients, no clear underlying cause is identified.
Symptoms
Symptoms typically include sharp, stabbing, or throbbing pain in the scalp, upper neck, or back of the head. The pain may occur on one or both sides and often follows the path of the occipital nerves. Episodes can be intermittent or continuous and may be triggered by routine movements, such as turning the head, brushing the hair, or applying light pressure to the scalp. Some patients also experience scalp tenderness, sensitivity to light, or aching pain behind the eyes.
Diagnosis
Diagnosis is based on a thorough clinical evaluation, including patient history and physical examination. Physicians assess the location and pattern of pain and may apply pressure along the occipital nerves to reproduce symptoms. Imaging studies, such as MRI, may be used to rule out structural causes. In some cases, diagnostic nerve blocks are performed, as temporary relief following injection can help confirm the diagnosis.
What are the treatment options for occipital neuralgia
Treatment typically begins with conservative approaches, such as medications to reduce nerve irritation and inflammation, along with physical therapy to address contributing muscle tension. Occipital nerve blocks can provide both diagnostic information and short-term relief. For patients with persistent or severe symptoms, minimally invasive procedures, such as nerve ablation or surgical decompression, may be recommended to provide longer-lasting pain control.
