Skull Base Fractures
What are skull base fractures?
Skull base fractures are breaks in the bones at the bottom of the skull, an area that supports the brain and contains critical structures, including cranial nerves and major blood vessels. These injuries are most commonly caused by significant head trauma and can range from linear fractures to more complex patterns involving multiple bones. Because of the density of vital anatomy in this region, skull base fractures require careful evaluation and management.
What causes skull base fractures?
Skull base fractures are typically the result of high-impact trauma. Common causes include assault, sports injuries, motor vehicle collisions, or falls from height. The severity and pattern of the fracture often depend on the force and direction of impact. In rare cases, underlying bone weakness may increase susceptibility to fracture.
Symptoms
Symptoms can vary depending on the fracture’s location and severity. Common signs include bruising around the eyes or behind the ears, clear fluid leaking from the nose or ears, hearing changes, dizziness, or facial weakness. Patients may also experience headaches, nausea, or neurological symptoms if nearby brain structures are affected. In some cases, symptoms may not be immediately apparent and can develop over time.
Diagnosis
Diagnosis is primarily made through imaging, with CT scans serving as the most effective tool for identifying skull base fractures. MRI may be used to evaluate associated soft tissue or brain injuries. A thorough neurological examination is also essential to assess for cranial nerve involvement or other complications.
How are skull base fractures treated?
Treatment depends on the severity and associated complications. Many skull base fractures can be managed conservatively with observation and supportive care. However, more complex cases that involve cerebrospinal fluid leaks, vascular injury, or nerve damage may require surgery. Multidisciplinary care is often necessary to address the range of potential complications and support recovery.
