The Center for Population Health Management and Quality at UT Physicians is a cornerstone for population health management, guiding operational efforts to deliver equitable, patient-centered care with the highest quality and efficiency.

Innovative systems improvement and quality

Our goal is to align best practices for evidence-based care with population health management strategies; we strive to encourage better health outcomes and lower costs. We aim to optimize resource utilization within UT Physicians by streamlining operations, maximizing efficiency, and strategically allocating resources to support impactful initiatives. We foster a culture of continuous improvement and promote a data-driven approach to health care. To ensure a commitment to quality from an organizational perspective, we maintain a practice-level focus on population health management and safety. We also are dedicated to advancing patient safety and quality, which are fundamental pillars of our mission.

Interdisciplinary collaboration and knowledge integration

Our goal is to cultivate strong partnerships with both internal teams (e.g., researchers, IT) and external stakeholders (e.g., policymakers, community organizations) to foster comprehensive, sustainable improvements in health care. We aim to remove silos across UT Physicians by bringing together a range of expertise for effective problem-solving and knowledge sharing. By strengthening strategic relationships with research organizations, we strive to advance collaborative research, spark innovation in health care practices and policies, and ultimately enhance health outcomes for the communities we serve.

Analytics and custom applications

We aim to strengthen our data analytics capabilities to generate actionable insights, guide evidence-based decision-making, and assess the impact of interventions on population health. By collaborating with clinicians and researchers, we develop tailored software solutions that facilitate data collection and encourage patient engagement. Through comprehensive development services, we streamline processes, enhance efficiency, and ultimately improve health outcomes across our organization.

Health care access and healthy living

We strive to broaden health care access and deliver improved outcomes for every patient by addressing various factors that influence well-being. By emphasizing patient-centered care, we seek to foster an environment where all individuals can benefit from high-quality services and supportive resources.

Learning and QI/research support network

We provide hands-on training and guidance to new investigators, departments, and students, equipping them with the skills needed to conduct effective quality improvement and research projects at UT Physicians. This support fosters a culture of continuous learning and innovation across the organization.

Pilot innovative projects

We provide pilot funding and collaborate with faculty and administrative leaders on quality improvement or research projects designed to improve health outcomes for our patients, fostering innovation and tangible advancements in care.
